Different options for Hub or Locality boundaries formed part of the discussions which took place at the "Design Your Hub" Engagement sessions in September. At this stage three options were presented for discussion and feedback; Harrogate Town, Ripon & Rural, Knaresborough & Rural - these options were pulled together from detailed analysis of data and consideration of Primary Care Home characteristics. The Programme Board and Joint Management Team (JMT) reviewed the outputs of the engagement sessions and took a decision following these sessions to form two "Super Localities" for Harrogate and Ripon & Rural. This decision was based on factors including the distribution of primary care practices across Harrogate and Rural districts, and population size. Creating two Super Localities provides opportunity to segment into 2 smaller Localities (creating 4 in total); that align with the principles of the Primary Care Home model which is based on the provision of care to a registered population of between 30,000-50,000.
A map of the proposed Super Locality boundaries is included HERE. Within each Super Locality, it is proposed there will be two Locality teams in order to create a manageable geographical scope, and operational teams of c.50-60 colleagues. The options for where the Locality boundaries need to be drawn within the Super Localities is being worked through by a Working Group comprised of reps from each partner organisation; looking at factors including current operational team boundaries, activity data, distribution of primary care practices, and other factors such as location of residential care and nursing homes.
Over the course of the first year will will be looking at what opportunities there might be for joint roles and apprenticeships. It is likely that this will evolve as plans are worked up in more detail and we will provide more information as it becomes available.
